2016 was the deadliest year ever for migrants trying to reach Europe. More than 3,000 people died trying to cross from Libya to Italy. BBC Radio 5 live's Lucy Grey reflects on the 12 days she spent broadcasting live from a search and rescue ship as it pulled nearly 400 people from the sea near the Libyan coast.
