Overview: IV to PO conversion has immense potential to benefit patients by preventing unnecessary intravenous therapy associated with risks such as prolonged length of stay, line-associated infections, and excess cost and inconvenience. Although it is often listed as a low hanging fruit for antimicrobial stewardship, there are many nuances to cover.
