Yousuf Ahmad, President & CEO at AssureCare, has been working on the problem of population health for a while previously as a hospital executive and now at AssureCare.  Healthcare IT Today had the chance to sit down with him and talk about some of the market challenges healthcare organizations are facing and AssureCare\u2019s approach to solving them.  Plus, he shares what makes AssureCare\u2019s approach to population health different than other vendors.  Ahmad shares some of the proactive ways they are improving care for patients and he dives into how they\u2019re working with SDoH (Social Determinants of Health) to improve the health of populations.
